Heritage OnCare Home Health is seeking an experienced Medical Social Worker, on a casual basis, to join their growing health care team!

The ideal candidate has the ability to work in an independently, providing direct care in the client’s home setting. This individual will be part of a multidisciplinary team, working cohesively to attain the goals individually set for each client. The MSW will provide individuals and families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include, but not limited to, advising family, clients and caregivers with the knowledge and tools to manage their current state of health, as well as providing client education and counseling, and making referrals for other services in the community.

The agency provides all technology needed to complete the position’s tasks, (phone, tablet, EMR, internet access, etc). We offer competitive wages and mileage reimbursement, and our care is based in the Omaha metro area.

Responsibilities may include:

  • Knowledgeable of client/client legal rights, respects client/client rights always, and reports the failure of any other person to comply with client rights including abuse, neglect, or disrespect.
  • Maintains confidentiality about all client’s personal, financial, and health information including following HIPAA guidelines. Deals tactfully with team members, clients, families,
  • Knowledge of the Medical Social Worker assessment process related to home health and knowledgeable of Medicare regulation.
  • Must possess the ability to make independent client decisions when circumstances warrant.
  • Must be willing to seek out new methods and principles and incorporate them into existing social work practices.
  • Is knowledgeable and can easily access and implement federal and state homecare regulations as they relate to home care services. Demonstrates the ability to critically think, plan, organize, and problem solve needs of the department and takes the initiative to implement objectives resulting in favorable outcomes.
  • Understands and utilizes universal precautions, utilizes and maintains equipment as directed
  • Performs necessary documentation including: consent, evaluations, assessments, plan of care development, discharge planning and follow through, and order entry and adherence to, etc. in a timely manner and per agency policy.
  • Consults all client physicians, to ensure agency programming meets or exceeds their expectations and any orders they have given. Devises client specific realistic goals, with the physician’s approval, after the initial evaluation visit. Reviews the plan of care as often as the severity of the client’s needs require, but at least every 60 days. Informs the physician and other healthcare team members of changes in the client’s MSW POC.

Qualifications:

  • Graduated from an approved school of Social Work with a Bachelor’s Degree or Master’s Degree in Social Work.
  • Must be licensed as a Social Worker in the state of Nebraska and Iowa.
  • Must possess a current CPR certification
  • Three years’ experience as an MSW
  • Prior Home care experience preferred
  • Ability to provide care in a variety of settings
  • Must be at least 18 years of age
  • Be of good moral character
  • Not have been convicted of a crime under the laws of Nebraska or another jurisdiction, the penalty for which is imprisonment for a period of more than one year.
  • Speaks, reads and writes English language
  • Requires attention to detail in a fast-paced environment
  • Must be able to multi-task
  • Must be able to handle emotionally upset customers, be flexible and calm in stressful situations
  • Complies with all policies and procedures of the agency, including those specified in the team member handbook along with approved professional dress code.
